Here's an interesting thread over on "The Trench Line" for making your own gas mask record card: In the continuing spirit of "do-it-yourself" and for all those who would like to try their hand at it, I have created a series of printable templates for the Small Box Respirator Instructions Card and Sleeve.
To make this yourself, you'll need the following items:
(1) Sheet of "canvas paper" - 8.5" x 11" Available at craft and art supply stores, or online. Make sure the canvas paper has a canvas texture on one side and is smoother on the other.
(1) Sheet of manila card stock - 8.5" x 11" - (105lb. weight or more) Available at office supply or craft stores. You can also cut down a file folder.
An ink-jet printer Highly recommended. You'll get a cleaner result.
(2) Brass Eyelets (5/32") and setter Craft and fabric stores carry these.
A circle cutter or 1" circle paper punch You can also use a nickel as a guide, if you're careful.
A tube of white glue, any brand.
